New Clients

Please fill out this Registration/Medical History form and bring it with you to your first office visit.

Our new client consults go above and beyond the typical ‘office visit’. Many clinics will hurry you in and out with only a few minutes to spend with the doctor. We will not only provide a comprehensive exam with advice on all areas of your pet’s care, but you will have ample time to ask any questions you may have specific to your pet.

We will provide the following for every new patient in our clinic:

  • A complete review of your pet’s medical history including examination findings, medications, laboratory work, vaccine and dietary history. Former records are an important part of your pet’s evaluation and we prefer to have them before your visit to allow for adequate time to review the information. Please send records at least 72 hours before your appointment, or if you prefer, we can call your former veterinarian(s) and have them sent directly to us.
  • Complete physical exam
    Your pet will receive a complete examination of all body systems, including eyes, ears, mouth, teeth and throat, lymph nodes, skin and hair coat, heart and lungs, and abdomen. In addition we will feel for any lumps or bumps, evaluate your pet’s gait and check the joints and bones of the legs.
  • Dietary recommendations specific to your pet
    Do you wonder if you are feeding your pet the right food? We will help you evaluate your pet’s current diet and make recommendations based on age, breed, activity level, as well as any health conditions. We will work with your budget and lifestyle to create a healthy diet for your pet that may include nutritious commercial diets, home-cooked or raw ingredients.
  • Vaccine protocols
    Every pet has a different risk of exposure to disease. Our goal is to give your pet only the vaccines that are absolutely necessary for its protection. In addition, we offer alternatives such as titer testing and vaccine nosodes. When traditional vaccines are given, we offer detox protocols to help minimize side effects.
  • Behavioral advice
    Does your pet’s behavior leave you baffled at times? We have some natural remedies such as flower essences and essential oils that can help modify behavior and emotions. If additional professional advice is necessary, we have a number of referrals to behaviorists and trainers.

Payment Options

1. Payment is expected when services are rendered.

2. We accept debit cards, credit cards (Visa, MasterCard, Discover) and cash. We do not accept checks for first time clients.

Emergencies

During our regular office hours, we are always available to accommodate you and your pet should a sudden injury or illness occur. If possible, we ask that you call ahead so that we know to expect you. For after-hours emergencies, we refer to the following quality hospitals:

Sick Pet Exams

These exams are for pets that are having symptoms but are not critically ill. We will complete a full physical exam and recommend diagnostic tests as indicated for your pet’s condition

Wellness Exams

These are regular check-ups that we recommend every 6 months. We will perform a full physical exam, review diet recommendations, and answer any questions you may have about your pet’s health. You see your pet every day and sometimes changes happen so gradually that they are overlooked. We will help keep your pet in optimal health through all stages of its life.
